2024-004's wound on the right side of the body was examined for closure and possible suture removal. On exam of the area, the wound had closed margins, so sutures (6) were removed and TMS was discontinued. A band (gold R022) was added for additional identifictaion of 004.

Plan: Move 004 to flight enclosure and monitor for molt or possible impimg of feathers to the left wing to prepare for release.

Comments
Prior to taking radiographs, the patient was placed under anesthsia using iso via mask. Thoroughout the procedure, the patient had its heartrate and repsiraotry rate monitored to ensure it was stable.
On radiographs, the pateint had a mild pnuemocoelom. When viewing the shoulders region, both shoulders appeared symmetrical along with the soft tissue structures. There was also no evidence of any broken ribs.
Following the completion of radiographs, the pateint was woken up from anesthsia and vitals were still monitored until it was wake and able to support itself.
Moving forward, this patient should be kept quiet and on cage rest to allow time for healing. The wound present on the right side of the body should continue to be monitored for healing every 2-4 days to ensure closure and evaluate the sutures. The right shoulder can also be palapted during this time as well to acess any changes or discomfort. Currently the plan is to continue TMS for the wound in terms of medication.

------------- PHYSICAL EXAM FINDINGS ----------------
Attending Veterinarian: Dr. Seth Oster
Observational examination
Mentation: Bright, alert, and responsive
Posture: Normal - No discomfort noted
Overall appearance: Normal
Vital stats
Dehydration: Euhydration
Physical examination: Burn wound present on right side of body - previous debridement and closure of wound
Oral cavity: Normal
Beak: Normal
Cere/nares: Clean
Eyes: Clear
Ears: Clear
Feather quality: Multiple burned feathers: Left wing primaries
Wings - Range of motion: Right wing at the level of the shoulder had abnormal rotation on palapation
Legs/Feet: Normal
Legs/Feet - Range of motion: No discomfort on manipulation
Legs/Feet- Talons: Burns located on dorsal surface of talons
BCS: 3
Droppings: Normal dropping present in the morning
Other observations: Pateint has been eating well during feeding.
Problem list: 1. Burned feathers on left wing
2. Wound closure following the debridment of the burn on Sunday March 10, 2024
3. Burn on talons
4. Abnormal rotation of the right shoulder joint
Diagnosis: Burns are most likely a result of electrocution. The abnormal rotation of the right shoulder could be due to a soft tissue injury or a fracture in the coracoid region.
Assessment: The wound closure on the rigth side of the body appears to be healing well with only minor gaping present when the closure was palpated. The sutures are also still intact. With the abnormal rotation of the right shoulder joint, this could be a result of a fracture in the coracoid bone or possible soft tissue injury.
Plan: 1. Continue monitoring the burn wound closure on a 2-4 day basis to make sure suture remain intact along with how the wound in closing up.
2. Continue TMS until the wound is fulling closed to help elimate any secondary bacterial infections
3. Radiograph the shoulder joint to evaluate for injury
Prognosis: Excellent

Debridement and closure of wounds

Premed: Midazolam 2mg/kg, 1/2 given IM 1/2 given IN and Dexmedetomidine 0.02 mg/kg IM at 10:53 am. HR=230 bpm, RR=20 bpm
Intubated with 2.5 sized tube. Anesthesia with Iso, doppler placed on left ulnar vein, used heating pad under patient.
Used aseptic technique with dilute betadine and sterile saline to prep patient for debridement and closure.
Found 3 open wounds after removing dead skin - one along the right axiallary area, and the other two along the ventromedial wing between with elbow and shoulder. Dissected out necrotic and dessicated tissue using blunt and sharp debridement. Flushed each site with sterile LRS. Wounds were closed using 4-0 PDS, two of the wounds were closed using simple interupted. The third more distal wound was closed with simple continuous.
Heart rate dropped to 90 bpm during the procedure, even after lowering the Iso to 1%. Therefore, 0.02mls of atropine was given at 11:31 am. Heart rate remained at 90 so 0.02mls of Atipamazole was given IM at 11:34 am to reverse the Dexmedetomidine. Heart rate increased to 110 bpm and remained stable for the rest of the procedure.
At 12:18 pm 0.38 mls of Flumazemil was given IM. Final patient temp was 103.2 degrees F.
At 12:20 patient woke up and was moved back to cage. In the cage 2024-004 was standing with head down and acting as expected for post-anesthesia.

Wounds were examined and blood was drawn for evaluation. No change in status of singed feathers. The left first digit also has a burnt area on the talon. An area of burnt feathers and skin was also noticed on the right proximal leg.

The right jugular vein was attempted, but collapsed when trying to draw blood and only a minimal amount of blood was taken. After attempting to hold off the vessel a hematoma did form. The left jugular was then attempted and blood was successfully drawn, 2.5mls. The right axillary area was then examined. It has areas of burnt skin and feathers. Some of the dead skin and burnt feathers were removed and two small circular open wounds (approximatly 1cm in diameter) were noted. These were manually debrided and flushed with diluted betadine/sterile saline. Sterile lubricant get was placed on the wounds, which were then covered with tegaderm+telfa wound dressing.

Plan: wound debridement and closure under anesthesia tomorrow

Unwrapped right wing and examined the right axillary area. There were burnt feathers and skin- some burnt feathers were pulled or cut off and some dead skin was removed. This was left unwrapped and unmedicated to continue declaring devitalized tissue. The right and left wing appeared to have normal range of motion. The left wing had some burnt primary feathers and the some of the left tail feathers were burnt, but no damage to the skin near these areas. Normal mentation. Normal hydration examined at the eyelids and normal mucus membranes examined at the vent.
Will recheck in 24-48hr.
Future plan involves imping the burnt feathers on the left wing and tail.

Problem list:
Burn wounds/eschar of large area of skin
Burned/damaged feathers
Dehydration

Diagnosis:
Likely electrocution

Assessment:
Patient presented alert, able to stand on own. Large areas of skin and feathers are burned but not currently open wounds. Primary concerns are pain and infection so the patient will be started on meloxicam for inflammation and buprenorphine for pain. Antibiotics will be started to prevent/treat infection. The wound will need surgical debridement once it declares itself. Manuka honey can be used topically once wound is open using a body bandage. Dehydration will be corrected over 24 hours with SQ fluids.

Plan:
55mL SQ fluids q12h - maintenance + 7% dehydration
1 mg/kg meloxicam (0.18mL) SQ q12h
0.1mg/kg buprenorphine (0.05mL) SQ q12h
Hospitalization for wound monitoring
Prognosis: Guarded
